STP Super Liability reporting if employer contribution quarterly cap exceeds

Please clarify the query from the below example:

 

If the employer wants to continue in excess of  super quarterly threshold, then what will be output of below example.

 

If in case, if the employer accepts and employee wants to continue their Super even if the value exceeds the Quarterly Threshold Limit, then how and where the value should be reported?

 

Example:

Continue After threshold: YES

Luke is Paid: 67,000 (17000 + 50000)

Value will get calculated as 6365 (67000 * 9.5%)

 

Will this amount will be shown under Super Liability or Reportable Super Contribution?

 

Until Quarterly Threshold Limit to be reported under Super Liability?

 

Only the value over and above Quarterly Threshold Limit to be reported under Reportable Super Contribution?
