Based on the information you've provided, it sounds like you had appropriate hospital cover, but only your registered health insurer can tell you if your health cover was at the appropriate level for the whole year. An appropriate level of private patient hospital cover is cover provided by a registered health insurer for hospital treatment in Australia which has an excess of: - $750 or less (for a policy covering only one person), or - $1,500 or less (for all other policies). If BUPA advise you that your health cover is at the appropriate level for the whole year, then you need to answer Yes at label E item M2 to be exempt from the Medicare levy surcharge.
