My husband is fully exempt from ML and therefore MLS as a DVA Gold Card holder, but our combined family income is greater than $180k. This year our combined income is inflated due to DVA lump sums he has been paid. Do I still have to pay MLS on our whole combined income even though he doesn't? We have 4 dependents.

We look at both incomes, so if he is fully exempt under DVA he will put he is exempt at the Medicare for 365 days and complete the Medicare levy section accordingly. You still need to include spouse details and complete the medicare section for yourself.
The MLS will not apply to ADF members who are single and were entitled to full medical cover during the year. However, it will apply to ADF members who are entitled to a half Medicare exemption.
