Myself and my wife both are Australian Resident for tax purposes with combined Family income falling in the high income bracket . We have 1 child whose age is 2 years. We both parents have PHI with appropriate covers but child don't have PHI for complete FY21-22. Child was staying with us for complete FY 2021-2022. Do we have to pay MedicareLevy and Medicare Levy Surcharge

Whether you need to pay the Medicare levy depends on if you meet an exemption or not. Have a read through our Medicare levy exemption page to see if an exemption category applies to you. If it does, you won't be liable for the Medicare levy surcharge. You'll both need to check if you meet an exemption individually.
If you do need to pay the Medicare levy, you may be liable for the Medicare levy surcharge too. Based on what you've provided you both wouldn't meet the requirement of you, your spouse, and your dependants being covered by the appropriate level of cover for the year. You both will likely be liable for the surcharge due to this.
