I signed a contract to work remotely at home , there’s no near by main office. As a health professional I need to visit clients from time to time. My employer pays kms, but I’m instructed to deduct 35kms each way as this represents working at an office. Can you tell me if there is a rule for the 35km deduction even though there’s no office and I have a contract to say my work office is my home?

That is the employer's rule, not from ATO.
You can claim kms from home under these circumstances.
