If you have returning employees, that were terminated and now commencing again within 12 months, do they need to complete a new SC form as technically they are commencing employment.

Yes, under Super for employers - where to pay, it includes the following - You must pay SG contributions to your eligible employees chosen fund if they nominate a fund. An employee will generally tell you their choice of fund by completing a standard choice form.
There's also information under requesting stapled super fund details from us and when this may occur.
