Is a private health insurance that paid by the employer can waive the MLS?
Whether or not a Medicare levy surcharge applies to you is dependent on your income and whether or not you held an appropriate level of private patient hospital cover.
From the Medicare levy surcharge and your tax return webpage...
A Medicare levy surcharge may apply if you, your spouse and all your dependants didn't maintain an appropriate level of private patient hospital cover for the full income year.

Thanks for the response. For this scenario, where the combined income exceeds the family income threshold. Will the MLS waive if the employer provides private health insurance ? In other words, would the private health insurance provided by the employer be considered as an appropriate level of private patient hospital cover??
Who pays for your private health insurance is not relevant.
As per the above, the Medicade Levy Surcharge wouldn't apply if you have an appropriate level of private patient hospital cover.
@FinanceDude Okay got it! Thank you so much :)
