It is possible that you have been charged the Medicare levy surcharge because your family income is above the relevant threshold, even though your personal income is below the individual threshold.

@Glenn4802 is correct. The Medicare levy should be 2% of your taxable income, so if its more than that, it may be the Medicare levy surcharge. It should show the surcharge separately on your notice of assessment.
