Hi, I departed Australia in June and my visa expired at the end of last June too. However, I encountered difficulty accessing the DASP form due to a system requirement that mandates both a family name and a given name. Unfortunately, I have only a single-word name, which has caused an obstacle in completing the form. What should I do? Thanks

If you have a single-word name, you may experience issues using the DASP online application because the system requires both a given name and family name to be entered.
It may be best to submit a paper application instead. Where you send it depends on where your super is currently held.
If your super is held by:
- your super fund you can complete an Application for departing Australia superannuation payment (NAT 7204) and send a copy to each of your super funds
- ATO as ATO-held super you can complete the Application for payment of ATO-held superannuation money (NAT 74880) and send it directly to us.
Before submitting a paper application, it's a good idea to check what supporting documents are required and whether any documents need to be certified. If your super is held by a super fund, you may also wish to confirm any fund-specific requirements with them.
