Would a sole practitioner psychologist earning over $75,000 need to register for GST if all the services are GST free?.


If yes, then do the psychologist need to pay money to the ATO (say 10% of total income) even if he/she doesn't charge any GST from the client/patient?.

If you operate under an ABN yes. See this ATO link.

If your services are GST free (as some health services are) you don't send any GST to the Tax Office as your services don't include GST. You are entitled to claim the GST credits on expenses, which is an advantage.
