A supplier has provided a statement of supply to us previously, I wanted to check if they need to issue one for each invoice they provide us with or can we use a previous one? Is there a validity period for the statement provided to us?

A new form may not be necessary, you should keep sufficient records to identify the supply and the supplier and the reason for not withholding.

The statement itself must be kept for five years. This can be a signed statement or our form.
