Hi I was trying to complete my DASP application online, however every time I try and complete the form online I have been met with the error stating:

A fatal system error has occurred.Application Error A fatal system error has occurred. Details of this error have been forwarded to..

Could you please help me regarding this issues? because it is almost 6 months since I leave Australia. Thank you

Contact the ATO to get further assistance if you are experiencing technical errors using the DASP online application system
Take a screenshot of error and forward to DASPmail@ato.gov.au to enquire
Use paper application form (one for Superfund, and one if ATO-held super), if filling out form for superfund, the form needs to be sent to the relevant superfund.
