Hi Suresh,
If you have left Australia and your visa is cancelled, you can claim your super via departing Australia super payment (DASP) online. If your online application is successful, you wont need to provide any other documentation. The form you mentioned may be required if you apply using a paper application.

Many thanks for your email. I will submit my online DASP application with all required details.
I have a couple of more questions regarding the DASP.
1. I have not lodged my tax return application for the year I left Australia in 2016. Should this be done before I claim my superannuation via DASP? If yes, is it possible for me to do it now so that there is no problem when I am claiming the super?
2. Also, do I need to have a stamp on my passport from immigration while I was departing Australia permanently? Please note that I do not have such a stamp on my passport to confirm the permanent departure from Australia.
3. Is there any way to ensure that if I am claiming the super with a correct tax rate?
4. Is there a way to check beforehand how much would be my super claim before and after correct tax?
