I have tried several times to consolidate my super accounts by moving a small amount in an historic account (under my maiden name) to my current account (under my married name). Each time this has failed either because I cannot match exactly the "old details" on the transfer form (we are talking a job I had in uni in the mid-1990's) and/or they are not accepting evidence of change in name (a "certified" copy of the original). I thought using the MyGov ATO transfer portal would solve this issue, given my identity is clearly confirmed via my tax file number. The historic super fund provider is refusing to transfer funds without the requisite form (filled and rejected previously) and certified (within last 6 months) documentation. Is there any recourse available to address failure to transfer?

If you've provided required documentation to your super fund to update your name from maiden name to current surname, you may have to go through the complaints section of the fund. In the first instance however, they should also be able to advise you on what documentation will be accepted to complete the change of name request. Until your surname matches with your new fund, they most likely won't complete the transfer. You can view requests for further information to complete a transfer or rollover from here.

I think you need to correct the name on the old fund first - then there should be no problem.
Thanks Bruce4Tax. Unfortunately it is correcting the name on the old fund that is causing the grief - I've tried several times and for one reason or another it is not being accepted.
