If you're sending documents to update your legal name on our records, it’s best to sign the Section E declaration using your current legal name - the same name shown on the identity documents you’re providing as proof.


When completing the Change of details for individuals form to update your name, you’ll need to provide details from identity documents we can verify.


This may include documents such as an:

  • Australian birth certificate (full certificate, not an extract)
  • Australian marriage certificate, or
  • Australian change of name certificate.
