Client-Agent Linking - why does a client need to nominate an agent twice to give full access?

When you're adding the client to your list, are you using their TFN or ABN? As it depends on the client identifier that is used as to which account or role displays. If you're using the ABN, then the system won't show the income tax account option to choose. If you need to access to the income tax, then you will need ask your client to complete another nomination. To avoid this, I believe you should be using the business TFN this should give you access to all accounts and roles.
