How do I obtain paper form to register for Standard GST- Only

We are a foreign company selling software into Australia and need to register for Standard GST- Only as we also want the ability to reclaim GST. I am having trouble with the online application as it wants a TFN for any "associate" I am adding and will not let me move past this step without it. As we are a foreign company we do not have any associates with TFN numbers.non resident GST Registration It would appear that we may need to register via a paper form (see link).
