If my employee has had a change of surname what paperwork needs to be filled out please?
By them as the employee, by me as the employer and where would I find the necessary forms?

Whatever the written form of the request, you must sight the original document for the change of name before you can change the value in your system from the relevant date.
You then report the details in your next STP pay event, as you do normally. As an employer, that is the extent of your authorised action.
