I would like to know how to report my voluntary super contributions that were made via salary sacrifice during the 2017-2018 financial year.


Looking at my superannuation history and tax return for that year, I cannot see that these were lodged/reported in a way that I can determine the dates and amounts of the contributions, only that the total contributions in my super statement for that year exceeds my Reportable Employer Super Contributions on my tax return by the amount of my salary sacrifice contributions.


Can I just add a single contribution entry to FHSS with the date being 30 June?

No you do not do anything - it has already been done by your employer. To see the dates for your personal reasons you need to go back to your pay slips or ask your employer - the ATO will not get involved.
