we received a letter from my deceased mother-in-law's Super fund advising they had transferred two refunds for Advisor Service Fees to the ATO because Mum's accounts had been closed. The letter advises us to claim these from the ATO but I cannot find a form or any information on how to do this.
Can anyone advise if there is a form to do this?

You should be able to recoup this money via the ATO unclaimed super monies process:
Please note that to do this you will probably need to be the Legal Personal Representative of your Mother-in-Law's estate.
