I'm trying to set up my Centrelink account on MyGov. I've got MyGov set up with all my ATO, Medicare, MyHealthRecord, but have had to create the Centrelink account as I've never had one. During the process of creating/linking it asks if you've ever been known by another legal name. I'm married and changed my middle name and my last name. Because I changed more than my last name, I had to get a Change of Name Certificate (QLD) and I also have my Marriage Certificate (NSW).


Every time I've attempted filling this form in, with either of the certificate options, it cannot verify me. I can guarantee you I'm filling it in correctly. What is my next move? How do I fix this or move forward. I'm attempting to apply for parental leave but this has been hindering me for some time and I'm really not sure what else to do.
